Healthy Lifestyles Program

 
Diabetes education and prevention and chronic disease self-management education is available to individual community members, families or groups.  The Healthy Lifestyles Team can meet community members in the office, at home, in the hospital or at school/community venues.  They also act as a liaison between the community member and other members of the healthy care team, for example the physician, pharmacist or Island Health Diabetes Education Centre.  Diabetes education can include: Self-monitoring of blood glucose, healthy eating for diabetes, physical activity, blood pressure monitoring, medication education, etc.
 
Through the Healthy Lifestyles Program we provide : 
  • Diabetes Education
  • Nutrition Counseling
  • Chronic Disease Self Management
  • Good Food Box (Uy' Tsulhtun Le'tsus) once a month
  • Foot Care Nurse
  • Exercise classes
  • Cowichan Sweaters team for the Times Colonist 10K annually
 
Our team consists of a Registered Dietitian and Registered Nurse.
